1. ACKNOWLEDGMENT OF RISKS

I acknowledge that participation in soccer, athletic training, and physical activity carries inherent risks, including but not limited to:

  • falls or collisions

  • contact with other players

  • impacts with equipment or turf

  • sprains, strains, fractures, or muscle injuries

  • concussions

  • weather-related conditions

  • other accidental or unforeseen injuries

I understand that these risks exist even when proper coaching, supervision, and safety protocols are followed.

2. VOLUNTARY PARTICIPATION

I confirm that my child is voluntarily participating in programs provided by Vanguard Football Lab – Soccer Academy.
I understand the nature of these activities and accept full responsibility for determining whether my child is physically and medically capable of participating.

3. RELEASE OF LIABILITY

In consideration of my child’s participation, I hereby release, waive, and hold harmless:

  • Vanguard Football Lab – Soccer Academy (Ontario Registration #1001428820)

  • its owner, staff, coaches, and volunteers

  • the BGC Don McGahan Clubhouse

from all claims, demands, damages, losses, expenses, or liabilities arising from or connected to my child’s participation in academy activities — including injuries resulting from supervision or coaching negligence.

This release applies to all training sessions, scrimmages, events, and related activities.

4. INDEMNITY (PARENT/GUARDIAN RESPONSIBILITY)

I agree to indemnify, defend, and hold harmless Vanguard Football Lab – Soccer Academy against any claims made by:

  • my child

  • family members

  • other participants

  • third parties

that relate in any way to my child’s participation, behaviour, or injuries.

5. NON-REFUND CLAUSE REGARDING INJURIES

I understand that injuries, medical conditions, schedule conflicts, absences, or personal circumstances do not qualify for refunds once the program has begun, as outlined in the official Refund & Cancellation Policy.

6. CONFIRMATION & AGREEMENT

By signing below, I confirm that:

  • I am the lawful parent/guardian of the participant

  • I have read and fully understand this agreement

  • I voluntarily agree to all terms and conditions listed above
