Please complete and submit the following required forms:

• Liability Waiver & Assumption of Risk

• Medical Disclosure & Emergency Authorization

• Code of Conduct (Parent & Player)

• Refund & Cancellation Policy

• Media Consent Form

📝 These documents must be signed before your child can participate in training.

✔ STEP 2 — Complete Payment (If Not Yet Submitted)

Your child’s spot is only fully confirmed once payment is received.

U4–U6 Age Group — Payment Link
U7–U9 Age Group — Payment Link
U9–U12 Age Group — Payment Link

✔ STEP 3 — Prepare for the First Session

Your child should bring:

• Soccer cleats or athletic running shoes

• Shin guards (mandatory)

• Labeled water bottle

• Comfortable athletic clothing

Please arrive 10–15 minutes early for check-in and warm-up.
